Submit Visit Feedback
Rate a visit and send private feedback to the clinic.
Submit visit feedback when your clinic sends a feedback link after a visit.
Who does this: Patients and caregivers.
Steps
Open the feedback link from your clinic.
Select a star rating.
Answer each required follow-up question that appears.
Use Anything else? to add an optional note.
Select Send feedback.
Select Done on the thank-you screen, or continue to the public review option if one appears.
Common pitfall: Feedback links may expire after a set period.
Troubleshooting
The link is expired. Ask the clinic if they can send a new feedback link.
I cannot submit. Select a rating and answer any required follow-up questions.
