Review Portal Paperwork
Check assigned and completed paperwork in the patient portal.
Use portal paperwork to see assigned forms and confirm what has already been completed.
Who does this: Patients and caregivers.
Steps
Open the patient portal.
Select Paperwork.
Review Your Paperwork.
Check the progress summary, such as how many forms are completed.
Review each form status.
Select View on completed paperwork if you need to review it.
Open the intake link from your clinic if paperwork still needs action.
Tip: Completed paperwork can still be useful when you need to confirm what was submitted.
Troubleshooting
No paperwork appears. Your clinic may not have assigned paperwork yet.
I need to change an answer. Contact the clinic before submitting a new response.
