Send Intake Packets
Send paperwork to a patient from the Patients page or chart.
Send an intake packet when a patient needs to complete forms or consent documents before a visit.
Who does this: Front desk staff and clinic administrators.
Steps
Open Patients from the main menu.
Find the patient.
Open the patient actions menu and select Send Intake.
Select the packet to send in Send Intake to.
Choose Advanced: Select form manually if no packet fits.
Select Send Intake.
The intake is sent to the patient or guardian contact on file.
Common pitfall: If the patient has no valid contact details, update the patient record first.
Troubleshooting
No packets are listed. Create an intake packet before sending paperwork.
The wrong contact is used. Edit the patient details and confirm the patient or guardian contact.
