Build Custom Intake Requests
Choose individual forms and requests for a patient intake.
Build a custom intake request when a standard packet does not fit the visit.
Who does this: Front desk staff and clinic administrators.
Steps
Open Patients and choose the patient.
Open the intake action for the patient.
Select Manual Selection on Choose Intake Type.
Use Build Manual Flow to choose items from Available items.
Add the needed Form, Consent templates, Document requirements, Government ID, or Insurance card items.
Check the order in Patient flow.
Set how long the patient can use the link in Link Expiration.
Review the details, then select Assign Intake.
The custom request is assigned to the patient.
Common pitfall: A custom request only includes the items you select.
Troubleshooting
I cannot find a form. Ask an administrator to confirm the form is published and available for use.
The patient needs a standard packet instead. Go back to Choose Intake Type and select Use a Packet.
