Create and Publish Forms
Build a form and publish it for patient paperwork.
Create and publish a form when your clinic needs a reusable patient questionnaire.
Who does this: Clinic administrators.
Steps
Open Settings.
Open Templates under clinical workflow settings.
Select Forms.
Select New form.
Enter or edit the form name.
Add questions from the question palette.
Select Preview to test the patient view.
Select Publish when the form is ready.
The form can now be used in intake packets.
Common pitfall: Save a draft if you are not ready for patients to use the form.
Troubleshooting
Publish is unavailable. Add at least one question and check required fields.
I need to change a live form. Open the form and edit a draft before publishing changes.
